Mind Mapping allows people to more clearly understand a problem. Root Cause analysis forms a methodology for

finding and correcting the most important reasons for performance problems. This combination allows an individual to “see the whole” of a problem and fix it so it stays fixed. Attendees will learn to use Mind Mapping to summarize a project, subject or problem. The summary allows all of the factors to be visualized and their relationships understood. Attendees will then learn to use Root Cause tools to dig beyond typical initial answers, get to the true causes and find solutions to problems.

Problem Identification

  • SIPOC (a method to define and document Suppliers, Inputs, Process, Outputs, and Customers)
  • 5 Whys (a question-asking method used to explore cause/effect relationships)

Problem Correction/Prevention

  • Ishikawa (Fishbone) Diagrams
  • Cause and Effect Matrix
  • Tools for Corrective Action and Preventive Action (CAPA)
  • Benefit-Effort Matrix
  • Standard Work
  • Poka-yoke (a device to prevent defect production)

Students should bring a problem to class to use during the exercises. At the end of class, solutions can be taken back to the workplace and implemented.
